You will receive your online paperwork right after booking your appointment. Please fill this out at least 24hrs prior to your appointment.
Insurance verification will be done as a courtesy if requested but patients are responsible for knowing their coverage prior to the visit. We are no longer accepting new patients with Medicare, Aetna or United Healthcare.
LOCATION:
We are located in the heights with on street parking. Please don't come in without texting (971 258 1802) upon your arrival. We have an open concept office and may be with another patient.
SCHEDULING:
For future appointments you can book online @ amylesliedc.com. If you know you may need follow-up appointments you can book those in advance as my schedule does fill up quickly. There is a $50 no show fee for appointments cancelled within 48hrs of your appointment.
Please bring your insurance card and wear comfortable clothing that allows for movement.

How much does one Appointment cost?
If you are not covered by your insurance and you are paying out of pocket we offer a 10% discount. New patient visits are between $130-$185 and follow up visits are between $50-$80 depending on how detailed the treatment is. Cash or check is preferred as credit card fees are hard on small businesses.
Many other chiropractic offices don’t work with insurance companies to avoid the hassle of it all.
We believe that you already pay a lot of money for your healthcare so we want you to get as much out of it as possible. We also believe that it is important for us to work within healthcare to change their policies to recognize the value of our services.
Healthcare Insurance is a complicated world. We would like to think we know insurance plans in and out, but we are surprised by them often.
